Physiological Intracytoplasmic Sperm Injection (PICSI)

In conventional ICSI, sperm is selected based largely on its appearance and movement. While these are important, they do not always reflect the sperm’s genetic health. PICSI introduces an additional step where sperm are selected based on their ability to bind with hyaluronic acid – a substance naturally found in the outer layer of an egg. Only mature and healthy sperm have this capability, making them more likely to result in normal fertilisation and embryo development.

PICSI helps reduce the risk of using sperm with DNA damage, contributing to better embryo quality and pregnancy outcomes.

Why is it done?

PICSI is usually recommended in the following cases:

  • Repeated IVF or ICSI failures
  • High sperm DNA fragmentation
  • Poor embryo development in previous cycles
  • Male factor infertility with abnormal sperm morphology
  • Unexplained infertility in couples undergoing ICSI

This procedure helps embryologists take a more natural and informed selection, increasing the chances of using the most competent sperm for fertilisation.

The Process

PICSI is carried out in the embryology lab, immediately before ICSI. A special culture dish is used which contains droplets of hyaluronic acid. A sperm sample is introduced into the dish, and over time, only the mature, high-quality sperm will bind to the hyaluronic acid.

Our embryologist then selects these bound sperm and injects one into each mature egg, just like in standard ICSI. The fertilised eggs are then cultured and monitored over the next few days. Embryo transfer follows, and remaining good-quality embryos may be frozen for future use. PICSI adds just one more step to your ICSI cycle, but it can have a significant impact on improving embryo viability and overall success.
